In this report, a 3 year old boy with pancreatoblastoma is presented. A large abdominal mass extending from upper left quadrant to the umbilical region, hepatosplenomegaly and elevated serum levels of alpha-fetoprotein, alpha-1-antitrypsin and alkaline phosphatase were present on admission. The diagnosis of the pancreatoblastoma was established by the examination of the biopsy sample which showed tumoral islands consisting of epithelial cells with acinar differentiation and squamoid corpuscles scattered in benign fibrous stroma. The tumor was unresectable. Two courses of chemotherapy produced a dramatic regression in tumor size as well as in tumor markers. Whereas, the following courses did not further improve the response. It was planned to add radiotherapy, unfortunately, he was lost to follow-up. Owing to the limited number of cases of pancreatoblastoma, the clinical, laboratory and pathologic findings of this case are discussed here.
