Although not common in adults, thymomas are exceedingly rare in children and only a few well-documented cases are present in the literature. The authors report a case of a 12 year old boy who presented with malignant thymoma with distant metastases both in kidney and brain. Contrary to the slow course in adults, these lesions are much more aggressive and have a considerably poor prognosis in children as in the patient presented, who survived only 6 months following the diagnosis.
