Effect of L-asparaginase on glucose metabolism in children with acute lymphoblastic leukemia

Summary
Glucose metabolism alterations were investigated in 10 patients with acute lymphoblastic leukemia, treated with L-asparaginase alone for 10 days, after achieving complete remission. Glucose tolerance tests were performed in the patients prior to L-asparaginase therapy on the 4th day and the day after completion of therapy, with simultaneous measurements of serum insulin and C-peptide levels. Fasting hyperglycemia was not noted in any of the patients. Basal and glucose stimulated insulin and C-peptlde levels were consistently normal. However in 2 of the 10 patients, impaired glucose tolerance developed on the 4th day and became normal on the day after completion of L-asparaginase therapy. Our study suggested that L-asparaginase alone does not interfere with insulin biosynthesis or release. However L-asparaginase induced transient abnormalities in glucose metabolism may occur, probably due to the impaired peripheral utilization of glucose or developed insulin resistance.
